You don't ask her: "Do you want to be my girlfriend?" Or anything of the sort.
What you do is start asking her out and dating her. Once it's obvious that you two like each other and you've been dating for a while, a few things will start to change: (if it's a legit relationship)
1. You two will be exclusive.
2. It will be "us", not "you and I".
3. People will think of you two as a couple.
4. You will be fine with referring to each other as bf/gf when talking with others.
etc.
This is how you know she is your girlfriend.

I think it's good to give it some recognition though. Just asking them to be your girl friend is kinda weird, but most girls (not all of course) seem to like some kind of proposal. I remember I just asked mine something like "So are we gonna make this official or what" and she flipped out. I phrased it more suave than that though

my gf now was has been my best friend for a few years, we started getting closer and being more like a couple so i told her to get dressed up real nice, showed up lookin fresh to def...brought her out to dinner and on the way back asked her to close her eyes, slipped on a promise ring and told her i would love to make it official.
girls like cheesy ****, especially since most guys are either too pussy or think its too gay to do.

Im not saying your not right but thats not the case 100% of the time. I had the same thing going for me when i was in middle school, We hung out a bit out of school, i walked her to class and we would grab arms and sh!t, and kiss on the cheek (im innocent). Few of my friends even though we were going out, So did i. But about a week later she was going out with some baseball player, Swaping spit. Feels bad man, But just ask her out once you feel comfortable enough with her.
