    Question Sample contracts for personal trainers?

    Is there somewhere that I can find a sample or template contract for use with clients?
    Something that spells out each party's responsibilities, cancellation & payment policies, etc in a legally sound manner.

    I googled & didn't see much out there.

    I registered on NFPT but am having problems downloading - a site error comes up. I'll work on that with them later.

    For the forms I've seen so far (and thank you to all who provided responses), they mainly consist of cancellation policies & intake questionnaires (medical history, goals, etc).

    Are these the only forms that are typically used?
