You need to take a lot of weight off the bar and relearn the squat.
You are racking from too high, and taking it off the pins with your toes. You should be taking the bar off the pins with a '6 inch squat', not a calf raise. You should then be taking one solid step back into your squat position. Your feet need to be straightened out a lot, and your knees need to be pushed out more. You are also leaning forward and taking the weight on your toes at the bottom of each rep. Your weight should remain in the centre of your feet, or towards the heels.
Try this: Take a position on all fours on the floor and slowly push yourself back. Have someone check which point you go into butt wink. This is the low point of your squat, so you should be able to hit that depth with no butt wink.
Also, try dropping into a full depth squat with no weight. Can you sit there comfortably for a full minute?
