pain that prevents lifting above shoulders

i dislocated my left shoulder last year doing incline db press n popped it back in on my own. it wasnt a problem until i took a head to it playing rugby last fall. i stopped lifting shoulders because of the nauseating pain n weakness. i can bench without too much pain. i definitely cant lift any wt. above my shoulders. im wondering if anyone has an idea of what this is or any physical therapy i could do. would like to avoid doctors n surgery.
