Our gym is having an issue purchasing a full blown smith machine because of a "crushing" incident that happened a while back with someone not setting the safety pins properly on the smith machine while squating.
Because of that incident we need to purchase a smith machine with a bench (flat/incline adj) attached to the smith. Also, it would be very good to have the machine have safety catches that don't go below the bench.
Is there such a machine?

Most adjustable benches have holes for screws. Bolt it to the floor.Bodybuilding is 60% training and 50% diet. Yes that adds up to 110%, because that's what you should be giving it. Change the inside, and the physique will follow.
