Cartels.
Al-Qaeda doesn't have a solid network and really are just a bunch of small proxy groups spread all over the world. The cartels can thrive because political powers are scared ****less of coming down on them.

Cartels have their hands in the government. They have the best technology, and many of them own modern military vehicles and equipment.
In the real world whereas they have people inside governments including ours they have access to satellite images and military intelligence. On what level who knows, maybe not much but it's a heck of a lot more than what amounts to desert farmers have.
The only thing that those guys have is their willingness to kill themselves. That is a major threat but not enough to overcome money, modern technology and political power.
Cartels would suffer some losses but not much.

al quaeda has people willing to BLOW THEMSELVES UP in order to kill as many enemies as possible. Ill go with al quaeda. Al quaeda in afghanistan use actual military tactics of fire and maneuver, not just drive-by's like cartels do.
