First off the lockout is all triceps. Cable crossovers, flyes, etc. will not allow you to acheive a good lockout. It's all about the triceps. Different types of extensions and presses. As far as doing rack lockouts, they aren't sufficient. When you bring the bar down, you don't know what path the bar is going to travel. By doing rack lockouts, your lift is comprimised because of this. I would recommend doing 2-4 board presses; they allow you to have an eccentric and concentric phase of the lift. And pausing should always be used during the bench press, not just for certain phases.
