im asking because its part of my gym instructor course to design my own INFORMED CONSENT FORM
please read it and tell me what you think

this programe as been designed for health adults.
the program will have activities such as walking/running on the tread mill,the exercise bike,the cross trainer,rowing machines,
leg press,leg extensions,seated calf raises,standing barbell curl,bench press,dumbell side laterals,overhead extensions,wide grasp pulldown,crunches and reverse crunches.
i will also add other activities but i will let you know what they are before we do them.
these activities will improve cardiovascular fitness,strength in the muscles and also endurance and flexibility.
before you do any program you should do a warm up session before hand,also when you finish a program you should do a cool down session.
so this program will begin with a warm up session and then finish off with a cool down,the warm up and cool down will involve stretches,which will improve flexibility.
when you are doing your program all activities will be explained and demonstrated to you,if you get shown any exercises which you think you cant do or you should participate in please let your instructor know right away.
also when you are doing the program,at any time you feel dizzy,pain in the chest or ill,please stop what you are doing straight away and let your instructor know.
when your instructor as run you threw the program which as been set up for you please feel free to ask any questions at any time about
risks or benefits.
if your instructor explains something to you and you dont understand it please let them know and they will try and explain it easier for you to understand.
when you are doing your program you can stop the session any time you feel like.
Although the exercise literature gives overwhelming support to the benefits of exercise, there are some risks involved. The major health risks of exercise training are muscle and joint injuries
The rate of muscle and joint injuries among people who exercise on a recreational basis is not very high
The major health risks of exercise are small if the exercise program is initiated gradually and supervised properly. Overall, the benefits of exercise greatly outweigh the risks.
the program is designed to minimise any risks involved,
when you are doing your program at any time you feel pain or discomfort you should stop the program right away and inform your instructor,even if you think the pain or discomfort is nothing to serious you still should stop right away and inform your instructor.
i agree to participate in the program which as been specailly set up for me by my instructor,i understand for the program to be effective my instructor will change it from time to time.
i also accept that in any exercise there are risks of injury.
the purpose,benefits and risks of my program have been cleary explained to me by my instructor and i full understand everything,also i understand that at any time thre my program i can withdraw.
