Anyone have a shoulder problem where one is "rolled forward"?

Maybe due to training around joint inflammation, or due to a forearm muscle tear a year and a half ago but I have an impingement in my left shoulder. The front of the delt sticks forward more than the other and the side delt is much flatter than the other. I get pain where the front of the delt meets the bone on the top of the shoulder.
I had an x ray done and it looks like the head of the upper arm has moved slightly out and forward in the socket. The pain is not all the time and not really acute but the strength in certain motions is way down.
I have gone for one session of "active release" therapy on the shoulder and it seems to help a bit. I am looking for an exercise that will draw the head of the humerus back to where it is supposed to be.
It would be a huge fluke if anyone here had a similar problem, or maybe even knew exactly what I am talking about, but basically if you imagine the muscles are knotted up where the pec and front delt connect, that seems to be what is pulling the should forwarder. The therapist works to relieve the spasm there but all that does is stop the pull, it doesn't suck things back the other way. The only thing I know for sure is overhead pressing aggravates it, and lateral and bent over dumbell raises don't help. Any thoughts are welcome. I know it happened over a long period of time so I am not looking for a quick fix. What medical specialty should I see about this? Anyone?

I would find a Dr. or Therapist that is certified for Gratzen, this is a technique where they use stainless steal tools to break up scar tissue, I can say this works very well because the Chiropractor I work with uses this method and also teaches it in different states. I have seen many successful treatments where the pt has said, " I have been in pain for years till this" it usually takes 6 to 8 weeks of being treated 2 or 3x"s a week, but it's different depending on the injury. After your treated you need to do certain exercises to rebuild the tissue and strenghthin the muscle, Scapula stability, rot cuff strength and such.
Art is also very good but does not break up scar tissue like Gratzen (sp)
It would benefit you to look into this. You can google it,ask your Dr. Good luck.
