probably the first time you've heard this one
I'm new to the forums, but it seems like everyone who's discussed the whole breast issue has wanted to keep their current cup size while losing fat. I'm looking for the best way to decrease body fat in order to make them <i>smaller</i>.
I'm 5'8", small frame, about 170 at 35-40% bf (trying not to underestimate here) and a size 34DDD! (yes, it's all in the chest) Quite the problem, eh? Well this makes getting tops that actually fit around the boobs, playing sports, etc. really difficult. No disrespect to the girls, but they get in the way!
Right now I'm not really sure where to start, I'm sticking to keto at 1800-1900 calories (seems kinda high for a deficit to me) but I've hit a plateau and haven't really noticed much of a change in my cup size. Does anyone have any suggestions? At such a high bf %, do you think it would ever be possible to be a B or small C at, say, 15-20%?

Nah you aren't the first!  I actually hate having breasts myself so I have always tried to keep them as small as possible, even though I am at the opposite side of the scale as you!
Whether it is ever possible for your breasts to be that small really depends on how much of it is really fat. How big are the breasts of your mother or sisters? How early did you start getting breasts? Those sort of questions will help you guesstimate how much breast tissue you can actually lose e.g. my sister and I are both < A cups even though she is 5"10 and closer to 200lbs - very small boobs run in the family!

Wow, lucky! I'd love to have A's! lol. C's and D's run in the family, but my sister, mother and aunts are morbidly obese, so I don't have a great basis of comparison.
I suddenly got boobs when I was 12 or 13, and at that time I was probably 5'7" and 130 with B's (around where I'd like to be now, not sure if it's realistic or not). I started gaining weight when I started Advair and Zyrtec a few years ago, but I've been off it for maybe a year now and haven't been able to lose the weight...

I'm with you and Lou-Lou... Boobs are icky... Give me AA any day! So I completely understand you wanting to off the 34DDD...!!! Eep...
So.... Boobies are made up of two different tissue types - glands and fat... And Lou-Lou was correct in what determines boob-age size:
1. genetics
2. body fat
(3. hormone wackiness)
So, with the exception of changing your parents  (and in the absense of any hormone wackiness), the bestest way you can decrease what you currently have is to decrease your BF%...
In terms of why you can't lose weight -
1. Diet... diet... diet... diet... ermmm... and diet: Double and triple check your calories (keto tends to trick a few - due to the calorie density of the fats - so many underestimate just how much they are eating). Also - it may be that, as you say, 1800-1900 is too many calories for you - especially if you are not very active, if you are genetically 'slower', or and if you have a high % bodyfat...
2. exercise.... exercise.... exercise.... exercise.... So - are you training properly? Are you doing EFFECTIVE cardio? Are you active in GENERAL?
(there is also 3. hormone wackiness <--again)

But the last point is rare... And if you simply balance those first two things correctly. And repeat ad nauseum.. You SHOULD get results.

I guess I spoke too soon about the diet. I'd been eating 1200 calories for probably six weeks without losing much weight, so I bumped it up to 1800 for a week and still didn't see any change. But for some reason I stuck to it anyway and lost 5.5 lbs this week.
Anyway, I've done keto for probably four months now and have seen some other posts where people have said a low fat diet shrinks their boobs more so than low carbs. Should I switch to low fat, or keep at keto?
I've been to an endo to check for possible thyroid problems and that came back normal... should I have them check for anything else?
As for cardio... I used to run track, but I'm not sure I really want to try that with DDD's...
Sorry for being so long winded. lol

clearly, we want what we don't have - it's really true! i was the proverbial carpenter's dream (except for HUGE pec muscles i had little native breast tissue). so what did i do - i get implants. now i can't really bench heavy any more. benching and working pecs was my fave thing to do. also used to love to do heavy wide-grip pull ups with about 40 lbs around my waist. used to crank out at least 6-8 reps, 4 sets...not any more. i'm such a sissy now hehe. if they were over the muscle i wouldn't have any problems, but they are partially under.

it depends on your individual breast tissue / adipose tissue ratio in the breasts. The more breast tissue, the lower your chances to reduce breast size by dieting that will affect mostly the adipose part.
this could potentially aid in reducing the adipose portion of breasts:
Yohimbine. It is an alpha-2 adrenoceptor antagonist and increases noradrenaline release and cAMP levels. Increased cAMP is helpful for degrading fat in the cells.
But be careful to not overdose it, because anxiety is a common side effect of oral yohimbine.
I think that a topical solution with yohimbine applied directly to the breasts will be much better tolerable and give even better effects.

Sounds like you have been all over the place with your diet. Do you track your cals and macros? What is your training routine? You really should pick a diet plan and then stick to it for several weeks before making any changes. As Emma said, it's all about diet, diet, and diet.
I don't know why you would have gained weight on Advair and Zyrtec. I take Advair and Allegra and am hypothyroid. I did not gain anything by taking Advair and Allegra. It's good you got your thyroid checked and it's ok. That brings it all back to diet
