The only downside to a cortizone shot is that it weakens the tendons and ligaments in the surrounding area, leaving you more suseptible to a long term injury.

They don't look bad, but I would try to get more leg drive. This could prove to be difficult though because that bench seems to be really high off the ground.
IMO, widen the grip, that looks closer than my grip when I am doing close grip bench. Also it doesn't look like youre gettin much leg drive, focus on pushing your traps back into the bench.
