Very true, I've seen several video's of guys at PL meets using a thumbless grip and the weight slips off with 2 spotters on each end of the bar and one standing above them.
Strength is all about progression, if you made significant gains then there is no reason to start at ground zero. Start at a weight your comfortable with that you can do the 5x5 program with.
Don't over think it, if you're not gaining at the rate you desire, then up your calories by 200 for a couple weeks. You may find that you need to up it a couple times before you starting hitting...

There's no such thing as too low reps when building strength. The purpose is to continuously progress in weight. After my warm up sets, my last 3 sets are as such-
set 3:6-8 reps
Set 4:4-6 reps...
