The bar was forward before it even got to your knees. I know some peoples hips shoot up and it doesn't affect them but they shot right up on your 107 attempts and that might've had something to do...

For an oly squat shins should not be vertical and it is correct to let the knees go past the toes. Different coaches use different cues, not every coach agrees that the hips should be the first thing...
Look up Glenn Pendlay's 5x5, it's basically what you described.
Kolecki would do low bar squats, but he had more of a soccer players body than a weightlifter's body so I think it worked for him.
