I would say that it is better to have your knee on the bench for better stability. That way you are able to lift heavier. Also, you may want to try a variation which would be dumbbell rows lying...
I would change it to having one exercise per muscle group......maybe 2 exercises for a large muscle group like the back. That way, you would be able to complete about 10-12 exercises in one workout....
I would definitely aim for cardio just about from anywhere from as little as 2-4 days a week. Since you are already lean, you could probably get away with 2 for now and that will make muscle gains...
