What type of socks are you using? Cotton socks tend to give me blisters so I always try and use poly blends without cotton.

Also, is it always in the same spot? Could shoes be the culprit?

Fast enough for what? Are you trying to hit a tempo run, easy run, long run, recovery, etc.?

For me, how hard I'm breathing is a good proxy for determining if my pace is where it should be....

I've been using this method to increase mileage lately. Each step up in mileage followed by a step back week. Has been working well for me.
