For getting to 405, just add a few pounds every week, and eventually you'll get there. Try to work you're other body parts that work legs. Get in squats and other leg exercises, and you'll get there....

It really depends on how hard you want to work and how hard you work, if you train VERY hard, take like a 30-60 second break, if you want to get a good workout, don't take a break.
