top set, did it for more reps before... but kneewraps were lopsided so i looked like a ****ing panda bear

Your elbows pointing backwards are throwing you forwards.

Do the elbows really make that big of a difference? I never have the forward lean problem without bands...

Well judging from your forward lean in your avatar, and your elbow placement... just think of the physics of what your elbows do...

"5. Your elbows are not under the bar. If your elbows are pointed backwards (towards your butt) then you are sure to fall forward. Simply attempt to bring your elbows forward and under the bar. There is no exercise that can help correct this; just make sure you have verbal cues."
At the end on your way up you seem to be losing your arch - fixing your falling forward should definitely help that.
